Resident Survey Packet

Everything on one resident in a single document: demographics, behaviors with triggers, interventions (non-pharmacological first), medications with indications and GDR status, PRN use with effectiveness, rounding notes, compliance tasks with their rationales, risk scores, and adverse effects.

GDR Summary

GDR tracking facility-wide or scoped to one resident: status counts (overdue, due soon, on track, exempt), the compliance rate, and a per-medication table with indications, start dates, and last attempts.

Behavioral History

One resident's behavioral record: behaviors grouped by trigger category, the severity distribution, and intervention types with outcomes.

Facility Risk Summary

The executive overview for CEO, DON, and compliance-officer review: high-risk residents with scores, open compliance tasks by type, PRN-overuse flags, and missing-documentation counts.

An audited export history
